Classroom education on diet and exercise to reduce risk and impact of type 2 diabetes, followed by one-on-one counseling from certified diabetes educators customized to the needs and schedules of the patient weekly for the first 30 days and monthly thereafter. Our team of Certified Diabetes Educators inform patients about healthy eating, self-care, and assistance to manage contributors to Diabetes, including weight management, diet, exercise, motivation and milestones that prevent Diabetes in folks at risk for Type 2, or ease the struggle in managing Type 2 Diabetes for those who are already confronting this chronic health disease.
